    In order to make a Nationwide insurance claim, you will need the same information as you would to make a claim with any other car insurance company. All insurance companies require the same information from their policy holders. It is necessary to provide proof of your name, address, telephone number, date of birth and social security number. You will have to provide information about the date, time and location of the accident, and describe how the accident happened. Finally, if you were injured in the accident, then Nationwide will ask for details of your injuries. This will include what medical treatment you received and where the it was administered.
